  • This form MAY NOT be used for classes taken outside the College of Engineering.  Please download the form to your computer and complete it.

Master's Forms & Information

Email completed forms to eecs@uark.edu

  • Master's Committee Form - Complete this form and email it to the EECS Departmental Office immediately following or at the time of admission to the program of study.   This form consists of a major adviser and at least two other members of the current EECS faculty.
  • Guide to Thesis and Dissertations - The purpose of this Guide is to acquaint you with the Graduate School’s requirements for the format of your thesis or dissertation and explain the submission process.
  • Master's Thesis Title Form - Complete this form and email it to the EECS Departmental Office as soon as your thesis topic has been established and approved by your thesis director, but no later than three months prior to the date of the comprehensive exam.

Doctoral Forms & Information

  • Doctoral Committee Form - Submit this form to the EECS Departmental Office immediately after or at the time of the submission of the Declaration of Intent.  This form consists of a major adviser and at least two other members of the current EECS faculty.
  • Guide to Thesis and Dissertations The purpose of this Guide is to acquaint you with the Graduate School’s requirements for the format of your thesis or dissertation and explain the submission process.
  • Doctoral Dissertation Title Form - This form consists of the title of the dissertation and approval by the dissertation director.  Submit this form to the EECS Department Office at least ONE YEAR before the defense of the dissertation.
